Instructions for your Tuition Assistance Application

1. Complete the Parent Financial Statement form from 'School and Student Services'

• a. Complete either the Parent Financial Statement form (PFS) from School and Student Services (SSS) or the on-line version. You will need your completed 2009 taxes and SFWS's code, which is 7139.
• b. Make copies of your completed PFS, one to submit to the SFWS with your Tuition Assistance Application packet, and a second one for your records.
• c. Submit your PFS and pay the fee as soon as possible: If you filled out a paper copy, mail the original to SSS in the envelope provided along with a check or money order for $24. (Extra reports are $13 each—see the application for details.) If you submit your application online, you can pay the fee electronically.

2. Also complete these two forms:

• a. SFWS Tuition Assistance Application. All questions must be answered or your application will be returned, which will slow down your application process.
• b. IRS Form 4506-T

3. Put the three completed forms—a copy of your PFS, your SFWS Tuition Assistance Application, and the IRS 4506-T— into an envelope or folder with the following items:

• a. for both taxpayers: a copy of the completed 2009 Federal Income Tax package with all schedules
• b. for both taxpayers: verification of income—W-2 forms, 1099 forms, paycheck stubs, etc.
• c. a signed SFWS Statement of Intent
• d. a check for the non-refundable Tuition Assistance application fee of $50 per family

4. Return your completed Tuition Assistance Packet to the Business Office by 3/1/10. Please return ALL the items together in one envelope or folder. WE WILL NOT ACCEPT INCOMPLETE APPLICATIONS!

Although we strive to meet the needs of all our families, tuition assistance funds are limited. The sooner we have your completed application, the better job we can do to help meet your needs.

Apply to SSS right away, you will need a completed tax return: how quickly they process your application determines how quickly you hear back from us. If we need more information from you or would like to meet with you in person, we will contact you as soon as possible. Completed Tuition Assistance Packets are due at the Business Office by MONDAY, MARCH 1st.
