Tuition Assistance Checklist

Instructions for your Tuition Assistance Application

1.     Complete either the Parent Financial Statement form (PFS) from School and Student Services (SSS) or the on-line version (www.nais.org/financialaid/sss). After you have completed the form, make two copies of it—one to submit to the SFWS with your SFWS Tuition Assistance Application and a second one for your records. You will need SFWS’s code, which is 7139.

2.     Mail the original of your completed PFS form to SSS in the envelope provided as soon as possible along with the appropriate fee ($24.00)

3.     Complete the SFWS Tuition Assistance Application .
(Please be thorough! Incomplete applications will be returned, which will slow down your application process.)

4.     Return the completed SFWS Tuition Assistance Application along with the items below to the SFWS Business Office by February 29, 2008:

• a check for the non-refundable Tuition Assistance application fee of $50 per family

• a copy of your entire completed 2007 Federal Income Tax package

• verification of income (i.e. W-2 form, 1099 form, paycheck stubs, etc.)

• a copy of the completed Parent Financial Statement form that you have mailed to
  SSS 

• a signed SFWS Statement of Intent

• the blank SFWS Enrollment Contract that is attached to your Statement of Intent  (This Enrollment Contract will be filled in with the appropriate figures and returned to you to be signed after a Tuition Assistance award has been agreed upon.)

Although we strive to meet the needs of all our families, tuition assistance funds are limited. The sooner we have your completed application, the better job we can do to help meet your needs.

Apply to SSS right away, you will need a completed tax return—how quickly they process your application determines how quickly you hear back from us.
